It is difficult to tell the exact model that he is playing in this video but it is clearly a ESP E-II M-II. There are a few different variations of this instrument but Gallagher is clearly playing one of them.

In the music video for From Womb to Waste and subsequent shows this guitar can be seen. Zooming in on freeze frames of the 12th fret of this guitar shows the model number of V-401DX.

In this podcast John talks about using a TS9 with the EVH head for the recording of Wrong One, he says that in future albums he probably won't use it again as the guitar was very sensitive to noise and was a little too saturated.

During the Milkboy sessions and the tour around then John can be seen using an ESP M-II. It is hard to tell exactly which one it is becuase there are multiple variations but they hall of the same basic layout. FLoyd rose, EMG pickups, ebony fretboard, and neckthrough construction. Becuase it is an ESP and not LTD there is no model number on the 12th fret.
