On Reverb someone is selling Wylde's Gibson Gothic Flying V that Zakk used as backup for Black Label Society tour.
The MXR Zakk Wylde signature Overdrive pedal can be spotted on Wylde's pedalboard in this Dunlop promotional video at (3:05).
One of Zakk Wylde's signature paintjobs, the Vertigo, is exemplified in this Gibson Zakk Wylde Les Paul Custom. The design finally came to life after a mishap when originally designing his logo. "I saw the poster from Vertigo, the Alfred Hitchcock movie, and thought that would be fucking awesome," Wylde said in this Guitar World Magazine interview. "So I explained it to my buddy Max, who ended up doing Slash's guitar, but when I went down there for the photo shoot, I opened up the case and saw the Bullseye logo. I said, 'Dude, what the fuck?' I had drawn it on a piece of paper and everything, but it was too late anyway. So we did the photo shoot, and the rest is history."
Wylde finally got the design done correctly. "It came out cool. So there's the Buzzsaw, the Bullseye and the Vertigo -- which is like a Bullseye, but 21st-century-style," he said.
In this Guitarist Magazine video, Zakk Wylde shows off his Zakk Wylde Custom Gibson Les Pauls. According to Wylde, All his Les Pauls are equipped, "with EMG pickups, a shaved maple neck, [and] ebony fretboard," (5:40). Wylde also uses, "6000 fretwire. It's thick and its high," he says at (5:58), "so you can get a really good grip on those things."
On this of Dunlop's website, it lists some of Zakk's gear, including an MXR MC401.

In the "On The Road with Zakk Wylde" video by Guitarist Magazine, Zakk Wylde is seen using Groove Tubes 6550 Power Tubes in his JCM800 2203 amplifier.
Zakk has been wearing Red Monkey gear since 1995 including Leather vests, Leather Cuffs and Guitar Straps
Zakk Wylde has a Fender American Vintage '52 Telecaster Reissue in his collection, which he played during a performance at a European festival.
Zakk Wylde uses the MXR M134 Stereo Chorus, which features musically voiced Rate and Width controls, along with Bass and Treble EQ adjustments for shaping the chorus effect. The pedal's stereo outputs provide a wide tonal spread, as detailed by Dunlop Manufacturing.
Zakk has at least two of these, one white with the stock pickups, and other originally blank now with a mirror front plate and EMG pickups.
More recently, he also acquired a black and white bullseye EDF-1275.

Zakk Wylde "has had a Dunlop Rotovibe on his pedal board since 1988, whether it's to spice up a solo, add an overdubbed color, or just as a sound to inspire songwriting" the Black Label Guitarist says in this Dunlop Promotional video at (0:04).
The Limited Edition Zakk Wylde Moderne of Doom is based on the never released third member of the Modernist Series of the late 1950s—the Gibson Moderne. According to Wylde in this Music News article, "the design is from 1958. The Moderne came out about the time the Flying V and Explorer came out. All three of them were not big sellers at all. In 1958, when they came out, they were way ahead of their time with those guitar shapes." Wylde worked with Gibson to design the guitar. "With the Moderne, it’s oddly shaped, and I really like those guitars," he said. "I definitely wanted to modify it for the 21st century. It’s as thick as a Les Paul now. It has the Custom-style block inlays, the Floyd Rose, the mahogany body. They did an amazing job with that guitar."
Used in the early days with Ozzy, most notably on “Mama I’m Coming Home”.
Based on some of photos on Zakk’s social pages, he still has the guitar, and takes very special care of it.
Only instance of Zakk playing this guitar is a video recorded in 1994, during Pride & Glory UK tour (Zakk Wylde Lovin Woman – acoustic).
Wylde is spotted using his his signature MXR Black Label Chorus pedal in this Guitarist Magazine video at (3:00). "The choruses are on all the time," he says at (3:24).
He plays mainly EMG 81 and 85 set in all of his guitars.
In this EMG promotional video, Zakk Wylde talks about why he likes EMG pickups. For Wylde, it's "the spectrum of sound, the clarity, the punch, and nothing gets muddy. The fidelity of the sound, all the way from the bottom to the highs...Everything is just crystal clear," he says at (2:00) "Once I threw the EMGs in my Les Paul, it was like 'Oh My God!' It was like night and day," he said at (3:00).
As seen at (04:55) of this Guitarist Magazine Video "On The Road With Zakk Wylde," the Black Label Society guitarist uses a Dunlop Rotovibe® JD4S onstage. According to Wylde, he uses it "for that Robin Trower, Jimi Hendrix stuff." (4:55).

Zakk had the Perfect Connection preamp on his touring rack during the "No Rest For The Wicked" tour.
As seen at the minute 03.10 of the "Guitarist"'s Video "On The Road With Zakk Wylde", Zakk Wylde MXR EVH117 Flanger during his Tour. The pedal is used in his pedalboard, placed behind the main heads and cabs right after his signature overdrive and chorus and an MXR EVH Phaser. The MRX EVH117 Flanger is used for only one song, "Crazy Horse".
As seen at (05:20) in this Guitarist Magazine video, "On The Road With Zakk Wylde," the Black Label Society guitarist says that he uses all the stuff he endorses, like the Monster brand patch cables on his pedalboard.

Zakk used the Metaltronix head on the "No Rest For The Wicked" album and tour.
Wylde shows off his ZV Buzzsaw guitar in this Guitarist Magazine video. It's an "SG and a V combination," Wylde says at (7:15). Like his Les Pauls, the ZVs come equipped with a "maple neck, ebony fretboard, and 6000 fretwire. I keep them stock with what Gibson actually sends me," he said.
