Surprised that no one has gotten the correct guitar he uses. He sawed the two horns off of this Strat. It's a made in Mexico seafoam green/daphne blue classic series 50's Fender Stratocaster with a rosewood fretboard as opposed to the maple. Notice the vintage tuners which set it apart from most standard Stratocasters.

Connan Mockasin was captured how he uses the legendary transistor JC-120 Jazz Chorus amplifier. The JC-120 Jazz Chorus is one of the few guitar amps that can truly be called a legend. Roland’s long-standing flagship is universally recognized by pros as the benchmark in clean guitar sound, and its rich tone and famous built-in stereo chorus effect have been heard on countless popular songs over its long history. I am not sure about the exact age of the amplifier.

While the original submission correctly identified the pedal in question, I just wanted to add that at 3:47 you can clearly see that the pedal isn't plugged into a power source and there's no cable coming out of the output––lol.
